Cancellation Policy
Restoring Hope Home Health, LLC provides only personal care services to individuals enrolled in the program under the direction and supervision of the Registered Nurse. All staff (includes contractual) will be trained and/or licensed by the state of State and produce prior to service delivery a copy of applicable licenses and/or training certification. Restoring Hope Home Health, LLC personal home care services provide: 1. Assessment of the personal care needs of the consumer 2. Participation in the establishment and implementation of the consumer plan of service 3. Provision of care services as defined by the consumer plan of service 4. Detailed attention to all consumers served and comprehensive care options are provided in attempts to assist clients in achieving an awareness of other options and accept and maintain a healthy and improved quality of life is encouraged. This includes but is not limited to: Personal Care, Housekeeping, Home management, Non-Medical related activities, and Companion care. The program provides services to all individuals regardless to race, creed, color, gender, national origin, sexual preference, or criminal history. Restoring Hope Home Health, LLC 's Personal Care Services assist clients in their homes. The program provides and /or assists individuals with everyday living activities. A need for assistance exists when the client is unable to complete an activity due to cognitive impairment, functional disability, physical health problems, or safety. The client's functional level is based on the client's need for assistance most or all the time to perform the tasks of daily living in order to live independently. Taking and recording vital signs, if specified in the plan of service; recording and reporting to the supervisor any changes regarding the client's condition, behavior or appearance and documenting the services delivered in the client's record. ALL PERSONAL CARE SERVICES WILL BE PERFORMED UNDER A QUALIFIED SUPERVISOR/RN/AND NOTES OF CARE WILL BE DOCUMENTED IN THE CLIENT FILE. AN LPN OR RN SHALL BE AVAILABLE DURING ALL HOURS THAT PERSONAL CARE SERVICES ARE BEING PROVIDED TO CLIENTS. THE RN WILL MAKE A SUPERVISORY HOME VISIT AT LEAST EVERY 30, 60 OR 90 DAYS (NOT TO EXCEED 90 DAYS) STARTING FROM THE DATE OF INITIAL SERVICE IN THE RESIDENCE OR WHEN RESTORING HOPE HOME HEALTH, LLC RECEIVES A COMPLAINT CONCERNING SERVICES AND THE COMPLAINT RAISES A SERIOUS QUESTION CONCERNING SERVICES BEING DELIVERED.
